Prior to your appointment, you will fill out an initial questionnaire. This will give Lara important information to develop a personalized diet plan for your child and/or family.  Initial appointments usually run 60-90 minutes.

Payment is due at time of appointment. Lara will provide you with an invoice that will serve as official receipt of services.  It is the responsibility of the client to submit to insurance companies for reimbursement. Nutrition services may be reimbursed through flexible spending accounts.

CANCELLATION POLICY

Lara recommends at least 24 hour notification for canceled appointments. Failure to cancel with at least 24 hours notice will result in the client being charged in full.

Grocery Store Personal Shopping ($150/hr)

In-home Meal Preparation Training ($150/hr plus grocery expenses)
